Online Appointment

The online appointment request service is not intended for emergency, urgent, or same day situations. In case of emergency, call 9-1-1. Click on the link above, complete and submit the form when you are ready to request an appointment. Requests for appointments are handled via a separate secure server at Collom and Carney Clinic Association. Following submission of the online request, a confirmation email will be sent to you of your request. Please allow 2 business days for a response. Our staff will attempt to contact you by phone to schedule a time and date convenient for you. If we are unable to contact you, we will to try to accommodate your appointment preference submitted in request, however the availability of dates depends on our scheduling parameters. When your appointment has been completed, a final confirmation email will be sent to your email address provided with your appointment time. Thank you for choosing us for your healthcare needs.
